Fuzzy Dripping Blood, Still

Fuzzy, our 6year old male grey, has been having a discharge or blood dripping from his penis for over a month now. Initially, it was very red blood dripping and some pus. He had no trouble urinating and did not seem to be in any pain. He had the same issue, almost a year ago, to the day, and our vet thought he might have passed a stone or some gravel. That cleared up with one week of antibiotics.

 

He has been on amoxicillin for a week, then off, and the blood returned. Then, clyndimicin for a few days with poor results. Back on amoxicillin for two weeks, seemed better and then some blood again. Then we tried cephalexin, which seemed to work and he started licked once the bellyband was removed and he is bleeding again.

 

Sometimes it seems that the bleeding is just from the tip, so our vet is wondering if the urine is strong. I have added cranberry capsules to his food the last two days.

 

The tip of his penis is very red and irritated looking. A few weeks ago it actually looked swollen. With the bellyband, he does not try to get at himself. Externally we have tried bacitracin, amergel, and silver sulfadiazine. Right now I have done nothing for three days, except to soak him a little in the blue antibiotic solution.

 

I don’t think the blood now is the same experience as the very red dripping blood. This seems to an external issue. Dr. Newman has examined the shaft and found only a very slight irritation. She called in another vet in the practice, and she examined him as well and could not come up with anything. They would rather not biopsy the area.

 

Tomorrow I am bringing a urine sample to see if it is actually a kidney or bladder issue.

 

Do any of you have insights or suggestions to offer our vet? She has said she is at a loss. Thanks.

Take some swabs and culture them. It'll cost you something, and the antibiotics he's had might interfere with results, but just might get things headed in the right direction. Sending gentle scritchies and fingers crossed for your boy.

Okay... here are the first things that come to mind...

If it is frank red blood odds are it is not coming from the urinary tract. This can be a sign of a bleeding disorder that is showing up currently in his sheath. You could run a CBC to check platelets or test to check his clotting factors to make sure it isn't thrombocytopenia or other clotting diseases. Might start with a week of Amicar to see if it will stop. If he is a GH bleeder, nothing will show up on any of those tests, but Amicar will work.

 

Prostate hypertrophy or infection can lead to bleeding from the sheath. It takes about eight weeks of antibiotics to clear up a prostrate infection.

 

I have heard of dogs who have pushed foreign objects far enough up in their sheath where you can not palpate them. But they cause bleeding.

 

A small tumor or ulcer farther up could also be the cause.

 

If I couldn't find anything else I might try contrast imaging. They inject contrast material and then take x-rays as it flows into his kidneys, ureters and bladder. Stones do not always show on a regular x-ray. Ultra sound picks up all three kinds if they are large enough.

The vet might want to consider taking a sterile urine sample from Fuzzy's bladder, rather than just getting a urine sample from his pee. If there are signs of infection in his pee, that still doesn't tell you where the infection is. But if a bladder sample is clear, the infection is further down the line.

 

I had to do this with my girl at one point. Turned out her bladder was fine. (The hardest part, considering how much she'd peed that day, was getting her tanked up enough for there to be anything in her bladder--and then getting her to hold it there until the vet could see her. She started to squat while we were in the waiting room...)

 

The sterile sample from the bladder is drawn by needle. If Fuzzy's not upset by all the attention he's been getting to his boy bits, getting a sample shouldn't be a problem. Jacey didn't even blink. I have to admit, the sample-taking is probably harder on mom than on the dog.

Here's my thoughts-- first I would take him off of antiobotics for atleast 3 days then bring him in for a cysto to run a urine mic culture. Has he had a rectal to palpate his prostate?? I would even stop the chlorhex (the blue cleaning solution) cleanings for a few days and culture the prepuce.. Has radiographs or an ultrasound been performed yet?? I would make sure that at least a lateral abdominal radiograph is done-- you will be able to see the bladder and prepuce on one film.

He is off of the antibiotics since Friday morning. He is not getting any topical lotions. The issue now is really the pus. It is not thick, but definitely smelly. If he gets the opportunity, he will lick and that is when he bleeds.

That sounds like a sheath infection.

I have seen Dr. Newman do it. She will have to show me tomorrow. I have been soaking the tip. I want her to culture the discharge and have been reading about bactrim as a possible antibiotic to try.

 

This poor dog. First his back injury, then the deramaxx made him so sick, the fall off of the car steps, and now this has been going on for well over a month.

oh boy, I'm sorry you are going through this. To me, it does sound like a sheath infection, just like Eric has been getting on and off now for about a year and a half. However, he only had a little bit of blood once. I've been flushing the sheath and once you get over the fear, it isn't a bad process. :lol Eric tolerates it very well. :blush

 

Our vet did some research with some reproductive specialists and they recommended scoping the area if these infections continue to persist, as there may be a foreign body lodged in there. Another recommendation was to flush him weekly once the signs of obvious infection have ceased, as part of a normal routine. His prostate and everything else is fine too.

 

Eric's urine culture was normal, so it wasn't UTI or other nasties. I hope the same is true for Fuzzy!

I couldn't get an appointment today with Dr. Newman, so we are going tomorrow morning. I will post an update if I find out anything.

The vet showed me how to flush and I believe she said you can't be namby pamby about it :lol .....I did it every day for a week or so. Huston was on clavamox for it.

The problem is, I think I am namby pamby.
