It Just Keeps Getting (worse) Now-better!

Beau is home. Considering he had general and he's had general more times in his 4.5 years of life -- you wouldn't beleive it, he looks GREYT. I mean really good. He is moving a lot better too. She "bit the bullet" and gave him metcam- doesn't like to give it with pred and it seems to be making a difference at least for the time being.

 

She sent the films to Cornell. The cytology of the swelled leg went to be checked out. His fever was 103.8 and the leg was warm to the touch.

 

The foot that was casted is much better- still a little bruised, but much better.

 

Having him home with me just brings tears to me eyes. Even though his eating for today is not good (we document every single drop of food he eats calorie wise, time he eats etc. Where I normally am getting in between 1800 and 2200 calories a day, he only had 840 today which all considering is not bad. We might make it up tp 1300 before the night is over.

 

His weight was 61.9 Fantastic. He''s got to get better. Not really knowing what to do is a problem though.

 

BTW- I was wrong, I said the right leg was swollen, but it's his right foot/hock very swollen. That's where the cytology was taken from.

One of their brothers, Jicky jack (or something similar to that) died of cancer at the age of 2. When we got Beau, we started looking for littermates and were told this by the track he had last been on. I always get completely numb when I am bringing him in for x-rays, first a young sibling and then their mom...

 

so far so good. But we don't know WHAT it is. :blink:

Oh my gosh Robin I am so sorry to hear the latest news today. I am glad that Beau is home though and that his weight is good. I hope your vets can get to the bottom this. Poor Beau already had enough on his plate. :grouphug

Robin, I can't imagine how you must be feeling. You can't catch a break at all, it seems. I hope Beau's problems get figured out and he gets better and better. You all need and deserve a rest now !!! Hugs to all of you !

For the TBD testing, definitely send it to NC State. Tell them to test for all TBDs they can test for. Also, when you send the samples, have them send serum and whole blood. That way if they need to do a PCR test, the blood will be there already.

 

Hoping you can find whatever is causing all this for Beau. :grouphug

But you know what it's NOT! (I always try to be the-glass-is-half-full voice of reason.) Whatever "it" is, I hope it's just a transient thing - no meds, just time. I can only echo the common sentiments expresses here - I am so sorry this is happening. Enough, already! Sending prayers for a quick, simple answer to what's bugging Beau.

If the cytology you're referring to is fluid from a joint aspirate, that should be your ticket to a diagnosis. They can culture this fluid directly to r/o infection (including spirochetes and any other TBD), as well as a bacterial infection causing a septic arthritis. I most definitely agree with starting the doxycycline empirically while waiting for the analysis to come back. Can't hurt, may well help, and you'll usually know within a couple of days whether it's helping.

 

I'd also give thought to the fact that, with his IBD, Beau is already an "autoimmune" dog. Just as in humans, an autoimmune disease may present in other organ systems, and with what you've described as a migrating pattern of joint pain/swelling, this might be classic for an autoimmune (or immune mediated) polyarthritis.

 

Type III: - IPA associated with gastrointestinal disease (enteropathic form). The diseased gut may show an increased permeability to potential antigens which could stimulate the production of immune complexes. Hepatopathic arthropathies have been described as well.

The joint fluid analysis should show particular changes that go along with systemic inflammation. An immune mediated polyarthritis would also present with fever, lethargy, lack of appetite, and generalized owie-ness. BTDT with my iggie, who my vet and I were sure had disseminated Lyme. Months on antibiotics still resulted in relapse until it was pointed out by a specialist that it was the steroids giving her brief remissions, not the antibiotics. Her Lyme titre at Protatek was low positive (1:640, per their values) so we kept treating for Lyme, until repeat titres showed a decreased rather than an increase. A joint aspiration and careful re-evaluation of her symptoms confirmed the diagnosis of immune-mediated polyarthritis.

 

Can't hurt to mention to your vet the possibility of Beau's current symptoms being a part of the overall constellation of symptoms asso. with an autoimmune syndrome. (see above)

 

I apologize in advance for saying this and contradicting your vet, but I have to suggest that combining any NSAID with a steroid, especially in a dog with IBD and an already-compromised gut, is potentially disastrous. If Beau is having a great deal of pain, either the vet should bump up the steroids if she feels this is inflammatory, or add on narcotic pain meds for better pain control.

Oh gosh :cry1 I'm only just seeing this thread, all three and a half pages of it! I'm so sorry Robin. It must be absolutely heartbreaking to see your Beau go through this. Glad he looks a little better since coming back from the vet.
