I don't understand what this is..how could I have missed this? I had him in the vets 3-4 times before being referred to a specialists. I don't even know what kind of cancer, other than fast moving - rare - non-common to greys from the Dr. Clinical History: 4-6 week history of appetence & weight loss. Blood work reveals moderate anemia, severe hypoproteinemia & mild liver enzyme elecation. Liver is very mottled & irregular; spleen is very nodular on gross appearance. Liver: dark brown, edges are rounded. Multiple off-white and less irregular area noted. Cut surface is dark brown to bile stained. Spleen: dark brown. No distinct lesions or masses are noted. Stomach: off-white-pale yellow. No distinct leisons. Microscopie: Liver - specimen consists of sections of liver in which there are disseminated foci of histiocyte/fibrohistiocyte infiltrate of the sinusoids, expanding the sinusoids & disrupting hepatic cords; similar, cohesive aggregates of cells are found in hepatic and portal veins; extramedullay hematpoiesis and numerous pigment laden histiocytes accompanying the inifltrate; regionally extensive hemorrage; hepatocelluar individualiztion & necrosis; canalicular cholestasis , regional lymphatic & venous dilation. Spleen: Regional hemorrhagiv infarct, multiple histiocytic nodules composed of cells with abundant cytoplasm & vesicular oval to irregular, vesicular nuclei with variably prominent nucleoli; multifocal extramedullary hematpoiesis; siderotic plaques. Stomach: Mild lymphoplasmacytic mucosal infiltrate. Small intestine: occasional dilated & irregular glands and mild lymphoplasmacytic infiltrate in the lamia propria Lymph node: with diffuse sinus histicytosis with RBC's in sinuses Diagnois: Liver & spleen leisons are most unusual. The extensiveness of the histiocytic infiltrate in the liver sinusoids together with the finding of histiocytic aggregates in vessels warrants consideration of this lesion as a manifestation of a histiocytic sarcoma or neoplastic histiocytosis. The most current terminology for some of these lesions is macrophagic sarcoma, according to researchers at University of CA. Prognosis is guarded. We looked to see if we could find anything in our books and found nothing - did not do an internet search, it's probably the best thing to try. All I can find is that histocytes are a sort of scavenger white cell which cleans up cellular debris and microorganisms and are part of the system that produces an immune response. It sounds very serious, similar to a blood or lymphatic cancer, but I really don't know. Do they have a treatment for poor Dundee? Chemo? Did they tell you anything else? Your best bet is to look at the human side of things, perhaps a search at www.pubmed.com (Medline) or head to your nearest medical school and their library. If the diagnosis of histiocytic sarcoma was correct then that was a bad disease that is ultimately fatal. : ( The biopsy was unusual in that the person who read it even mentioned it and stated "warrants consideration" and not "is diagnostic for". In cases like these I would typically have lab send the sample somewhere else for a 2nd opinion. If it were in Ohio, I'd probably send it to OSU and have them read it and then consult with Dr. Couto their oncologist on how to proceed as this is a rare diagnosis. I don't have very much info. to share on this condition as it is rare... but this is what I could track down: Malignant histiocytosis is often characterized by the systemic proliferation of large, pleomorphic, single and multinucleated histiocytes with marked cellular atypia and phagocytosis of erythrocytes and leukocytes. These histiocytes are pleomorphic, large, discrete, markedly atypical mononuclear cells; nuclei are round to oval or reniform. Features of malignancy that are usually present include marked anisocytosis and anisokaryosis, prominent nucleoli, bizarre mitotic figures, marked phagocytosis of erythrocytes, leukocytes, and other tumor cells, and moderate amounts of lightly basophilic, vacuolated cytoplasm. The presence of multinucleated giant cells also supports the diagnosis. Malignant histiocytosis is a rapidly progressive, ultimately fatal, disorder that has been described in adult dogs, including Bernese mountain dogs and other breeds; an increased incidence of the disorder has been suggested in the golden retriever and flat coated retriever breeds. However, I don't think there is much that can be done, as I recall. Tyler was diagnosed with malignant histiocytosis at OSU in November 2006. He actually had it as a bone tumor in his leg, but scans of the rest of his body before amputation showed that his spleen was starting to show involvement, so we cancelled the amputation, took him home, spoiled him and let him go. As I recall, radiation and/or chemotherapy aren't really effective against histiocytic sarcoma, which is why we cancelled the amputation - he would've been sick again by the time he healed from the surgery. I'm sorry. It sucks. But Dr. Couto is the one to talk to about this.
