Guest NahlaGreys2 Posted December 8, 2010 Share Posted December 8, 2010 Hi everyone - I've lurked around the forum for the last 8 years and it has been a HUGE resource! I have 2 girls - Annie and Kota - who are 11 and 12, I rescued them back in 2002. I've been through the ringer with Annie over the last 2-3 months. Started off with what I thought was a UTI - but urinalysis kept saying no. Eventually had ultrasounds, xrays and today a cystoscopy to try and figure out she is frequently squatting and now showing blood in her urine. So far no stones, masses, tumors have presented themselved. Rushed her to the emergency vet this morning because she was just flat out dripping blood The procedure showed perforation in her urethra - has anyone had any experience with this? I need to have a longer talk with the doc tomorrow when I pick Annie up - but she seemed to indicate it wasn't something surgery could fix and it would mean a regimen of antibiotics/anti-inflammatories (not prednisone). The doc biopsied some of the perforated tissue and is sending it off to check for transitional cells so until that comes back hopefully negative, still nervous Quote Link to comment Share on other sites More sharing options...

tbhounds Posted December 8, 2010 Share Posted December 8, 2010 (edited) Would the vet admit that he/she perforated the urethra during the cystoscope or was the perf there before the exam. I have heard of a few hounds presenting with TCC (transitional cell carcinoma) but, usually TCC is found in the trigone area of the bladder. TCC is a nasty, aggressive cancer in dogs --- fingers crossed that the path report will show no evidence of that. I won't discount the possibility that the perforation occured during the Cystoscopy - however the doc said she used the scope to follow the active bleeding straight to the perforated tissue. Which is why I asked if there was a surgical fix for the bleed (ie. cauterize, stitches or other). Will keep everyone posted as I find out more - like TeddysMom said - if it will help someone else in a similar situation, I'm more than glad to help! They kept Annie overnight to be sure she wasn't having any problems urinating this morning. I'm hoping to speak at length with the doc about the procedure to be sure she didn't just stop once she found the bleed - but when ahead and used the scope to view everything originally planned (vagina/vaginal vault/urethra/bladder) to be sure the perforation wasn't the only issue. Yeah ok, she found the bleeder but what caused that? Source or symptom? Then it's on to wait with baited breath to find out the result of the biopsied tissue. Here's my Annie - she turned 11 in August. Here's my 2 babies. Annie is on the left, Kota is on the right (who turned 12 in July). So far I've had both my everyday vet and an internal specialist run more than 1 test to rule out UTI or bladder infections. The samples were either ones I collected at home, they collected at the vet or even a sterile sample pulled directly from her bladder (while they had her out for the ultrasound). As for the urinalysis coming up negative (were cultures done as well?), there are many threads here of pups presenting with apparent UTI's but negative results improving significantly on antibiotics. The tech is going to relay my question to the doc - why is she straining? If the perforation and the blood was just a symptom, we're still no closer to what causing it. In the meantime, they have her on an anti-inflammatory, some pain meds and are keeping her hydrated since she hasn't been inclined to drink or eat much (my poor baby!! ). Since apparantly you can't modify a post - just heard from the vet and the overnight staff said she peed a steady stream she should be good to come home tonight However the doc wants to keep her for the day just to see for herself that the medication is doing what they think it should. Not sure yet what they have her on - just that she'll have 2 prescriptions, one of which will have to be compounded? The chats with the staff member haven't been that detailed, she's only relaying information from the doc - so of course I have more questions when I sit and speak with her this afternoon. Guest NahlaGreys2 Posted December 10, 2010 Share Posted December 10, 2010 So I have the results from the cystoscopy and I misunderstood what the doctor said. She didn't say perforated tissue, she said proliferative tissue. The problem is in her urethra - there is a large amount of proliferative tissue that is inflamed. The doc wasn't able to get the cystoscope all the way through the urethra and up into the bladder because of the significant narrowing of the urethra. We are still waiting on the results of the biopsied tissue but it seems it's either one of two main things ... cancer or some kind of inflammatory disease. So until the biopsy results come back, she's on an anti-inflammatory (piroxicam) and a pain med (tramadol). She is also on a second medicine (misoprostol) which is to help support her GI health from any potential ill effects of the piroxicam (can induce vomiting and GI ulcers) along with max strength pepcid AC.
