Based on the bloodwork, I'd still be suspicious of low-grade, chronic bleeding from a GI ulcer. His albumin level was a little lower than last time, which can also be consistent with blood loss. And GI bleeding can also increase BUN. Given that his creatinine was also increased, the rise in BUN is probably more likely kidney-related, and anemia of chronic disease is definitely possible. But treating for a possible GI ulcer is easy and won't hurt anything. I'd definitely continue the Prevacid or Prilosec, and consider adding Carafate/sucralfate given as a slurry, and see if it helps him feel better and improve his appetite.
