Sneaky Pie - I'm Terrified

I have not been here in years...I have missed this community and now I'm returning in need of advice and hope ❤️

 

I took Sneaky Pie to emergency vet Wednesday night with lethargy, weakness, and bruising all over. Her PCV was 19%. All other blood work normal for a greyhound except values pertaining to red blood cells.

 

Started tests. Belly ultrasound: enlarged spleen but no obvious rumors or bleed outs. Chest X-ray: no tumors. Clotting test: platelets not behaving normally. Not sticky. Blood smear: abnormal red blood cells. Some the specialist could not even identify.

 

PCV down to 12%. Blood transfusion at 5pm. PCV down to 11% 4 hours after that transfusion! Her body destroyed every thing they put in her. Another blood transfusion to save her at 2am. PCV went up to 19%, down to 16%, now back to 21%. I'm thrilled with this small step forward.

 

Am told there are 3 possibilities. 1. Infectious disease/tick disease. 2. Leukemia. 3. By process of elimination - autoimmune disorder.

 

Waiting on spleen aspiration results and tick panel results.

She's being treated for all 3 possibilities right now. Including first dose of chemo. Have done additional ultrasound of heart (looks clear) and eye (bulging but she also has previous eye issues. No bleeding behind eye).

 

Is there anything I should be specifically asking for at this point? Just waiting for any answer from these tests. 😢

How scary - I wish I had vast experiences to share with you but hugs will have to do - the only other things I would have them check out is rhabdo - we lost my precious bleu to this - and usually brought on by stress - or addisons - which is what the e vet originally thought she had - they also checked for all of what your vet has checked -- hoping for a good update on sneaky pie soon - researching these two possibilities might keep you busy for awhile at least - lethargy a big part of both -- prayers with you

Good thoughts for Sneaky Pie. Have them check again for tumors. I don't want to frighten you but we lost two to, suspected, hemangiosarcoma of the spleen.

As soon as I get that pathology report in my hands, I will reach out to Dr. Couto. He was very helpful to me with my previous greyhound, Penny. Such a kind man!

 

I just went to visit her. Her PCV will be rechecked at 4pm. She was so happy to see me for about 30 seconds, then promptly fell asleep pressed up next to me on the floor on a cuddly blanket. Lol.

 

The vet on duty (this is a large referral/emergency vet clinic with many vets rotating shifts 24 hrs a day) felt that the first chemo treatment is what gave her this step forward. He thinks Leukemia is most likely at this point. But nothing definitive yet. They are hoping the spleen aspiration gives a clear result because they really don't want to have to draw bone marrow.

I'm hoping it is tbd. That seems like the best outcome I could hope for! These tests take a crazy amount of time to come back 😳. Will tbd make red blood cells into bizarre shapes and configurations?

I have sent an email to Dr. Couto. I'm reading that certain kinds of HS responded well in a recent study with a certain chemo drug. 433 day survival rate vs 59 days. I'm still waiting on a more detailed report on her diagnosis. I would do chemo to give her over a year of life if it is a quality life. I would not do it to give her 2 months 😢. Meeting with oncologist on Wednesday. Thank you all for well wishes and prayers!

OK I've got Dr. Couto on board. I'll start getting things to him tomorrow. He says he first wants to make sure that the diagnosis is correct because there are some things that mimic histiocytic sarcoma.
