KF_in_Georgia Posted August 19, 2011 Author Share Posted August 19, 2011 I didn't realize that when the vet "waved off" TBD, she meant that she'd already dealt with it. Sample sent off for a panel, and doxycycline IV started. Jacey had an ultrasound this afternoon to check for potential cancer in her abdomen, liver, spleen, etc. Everything looked good. The vet said that if I wanted to be really thorough, we could do chest x-rays, too. But Jacey's breathing has been fine all along--and sounds fine now--so we'll hold off on this. (I just wanted the ultrasound today because if there was cancer, I wouldn't be putting her through this fight.) She still a very sick little girl. Presently, she's almost completely blind because she's bleeding into her eyes; once everything clears up (thinking positively, here), her vision should be fine. She's also still bleeding some from her nose and mouth, and she may be bleeding into her brain. We'll just have to see how it goes. She threw up once very soon after she got to the e-vet, but she hasn't vomited since then. I can't stay with her, but I can visit if I avoid "busy" times: rounds are at 7am and 7pm, meds/meals in the evening are 4pm (don't know about breakfast). The next blood draw for testing will be at noon tomorrow. That'll be the first real sign whether the prednisone is helping. The vets at the clinic serve a five-day rotation, and we've hit the beginning of the rotation: the vet we had today will be there for the next four days. While I was there this afternoon, Jacey's regular vet called to check on her. I have no idea why the smushed-nose sleeping position is popular with her just now. You can see the black bruising on her abdomen. My miserable little girl. Everything else is fine: cells are present and properly shaped. Ultrasound showed no abnormalities in internal organs. It's useful that we have a full "senior wellness panel" of bloodwork run on her less than 60 hours before all hell broke loose; that gives us good "then" numbers we can compare to the "now". Just remembered I phoned in a refill for Sam's methocarbamol. Drove by that pharmacy three times today, and didn't think about the pills until I got home. Sam may need that refill by morning. Sam is doing fine through all this: every time I leave the house, he's muzzled; every time he's muzzled, he gets a cookie. According to Dr Couto's presentation last weekend at the 3G event, greyhounds are odd in that they can bleed internally despite a perfectly normal blood clotting tests. In fact, greyhounds can actually bleed AND throw blood clots at the same time. They have done some studies on greyhound hematology at OSU. Might be worth a consultation. Have they thought of trying Amicar (Aminocaproic Acid) to control the bleeding? Dr Couto swears by it as the miracle drug for greyhounds at risk of bleeding out. Since they've begun using it, they have greatly reduced the number of greyhounds they loose to bleeding. Of course, Jacey's bleeding isn't due to surgery so I understand it might be totally different. Just thought I would throw it out there. Good luck! JJNg Posted August 20, 2011 Share Posted August 20, 2011 An article about ITP has this statement: "Unfortunately, mortality rates of patients with IMT approach 50 percent: death is caused by severe blood loss, especially bleeding into the gastrointestinal tract, or bleeding into the brain..." I could have gone all day without reading that. Sending good thoughts that Jacey responds well to treatment and recovers quickly. I'm having trouble opening that article, so I'm not sure where that reference comes from, but it's not consistent with what most other vet sources say. ITP typically has pretty good prognosis, better than IMHA. Most sources I've found give a 70-90% survival rate. Here at home, she'll snuggle with me for a while, then get up and move to a bed by herself. Tonight, I was curled up next to her at the ER. After nearly an hour, Jacey tried to get up (standing on her IV line!). I figure she was about to go looking for a bed she could have all to herself, so I kissed her goodbye and left. I asked the night vet about Amicar. Amicar is used to help prevent blood clots from breaking down. The problem for Jacey is that she has no platelets--and thus no clots for the Amicar to help. Jacey gets another blood draw for testing about noon tomorrow. After we see the results of that, I'll talk to the vet about chest x-rays to see if cancer is in the picture. But if the prednisone isn't helping, there may be some hard decisions to make. Meanwhile, my girl has magic marker squiggles all over her belly. They traced her bruises so they can track whether they're getting larger. Tonight, at least, all the bruises are the same size as the squiggles. Guest sweetpea Posted August 20, 2011 Share Posted August 20, 2011 Oh, I'm so sorry you're going through this. I went through idiopathic thrombocytopenia with my Ridgeback Brodie a few years ago. That's when I learned that idiopathic means it just showed up outta nowhere, and for no good reason. I had my suspicions about what brought it on, he had one infection after another that year, each requiring a different course of anti-biotics, each antibiotic more powerful than the previous. (Flea dermatitis, then a UTI, then anal gland infection.) We finally got him un-sick, and these huge bruises show up on his undercarraige, and we're back at the vet. The white blood cells were so over-stimulated by the infections, they kept on attacking even when the infections were gone. We put him on a course of prednisone, pretty high dosage, if I recall correctly, and then tapered down to zero. It was approximately 2 weeks, and at the end, he was fine. Good luck! But she's not out of the woods. She might feel marginally less miserable...but that's about it. Next up: chest x-rays and more bloodwork. I'll report results later.
